In case an employee loses or damages his/her work permit, he/she must apply for a work permit again. A dossier of application for a work permit re-issuance includes: 

The employer’s written request for re-issuance of a work permit due to loss or damage in Vietnam under the Ministry of Labor; War Invalids, and Social Affairs regulations.

Procedures for re-issuance of a work permit due to loss or damage in Vietnam

Step 1: After the employee discovers that the work permit in loss or damage; or the information on the work permit has been changed such as full name; date of birth, nationality, passport number, date of issue passport; working location, workplace, the foreign national employee is responsible for reporting to the employer.
